Free Implant Consultations with X-Ray

The organizer of the action under the name “Free implant consultations with X-Ray”, hereinafter referred to as the “Promotion”, is Medicadent Dentistry Sp. z o.o., headquartered in Poznan at Piątkowska No. 110A loc. U2, 60-649, registered under the number KRS 0000934484 by the DISTRICT COURT POZNAŃ – NEW TOWN AND WILDA IN POZNAŃ, VIII ECONOMIC DEPARTMENT OF THE NATIONAL COURT REGISTER, NIP: 7811952554, REGON: 368200700, company’s share capital: 10,000 PLN.

§1

1. The promotion starts on 01.09.2023 and ends on 31.10.2023.
2. A person eligible to participate in the Promotion (hereinafter referred to as the “Participant”) is every adult who, during the Promotion, arranges by phone or in person at the Medicadent facility for an implant consultation (hereinafter referred to as the “Consultation”). T
3. he consultation will be free of charge once for the participant, provided it takes place during the Promotion. All possible additional tests necessary to properly conduct the Consultation are performed at the sole expense of the Participant.
4. For the Consultation, all possessed radiological documentation should be brought.
5. The organizer reserves the right that the pool of free Consultations under the Promotion is limited and may be exhausted before the end date of the Promotion.
6. The order of applications determines the possibility of benefiting from the free Consultation.
7. The participant is entitled to a free X-ray examination (does not apply to tomography) only on the condition of having the Consultation at the Medicadent facility during the Promotion and subsequently starting the implant treatment process at the Medicadent facility no later than 31.10.2023.
8. Pantomogram, tomography during free diagnostics will not be charged and will be made available to the Participant for inspection only on condition of starting the implant treatment process at the Medicadent facility no later than 31.10.2023 or upon condition of paying the full implant insertion fee in advance.
9. The promotion is carried out in the following offices:

• Medicadent ul. Piątkowska 110A, 60-649, Poznań
• Medicadent ul. Strzelecka 8A, 64-200, Wolsztyn

§2

1. Participation in the Promotion is equivalent to the acceptance of these regulations without reservations.
2. These regulations are public and available at medicadent.com.pl.
3. Additionally, the content of the regulations will be made available upon request to all potential Participants at the Medicadent facility.
4. The Organizer of the Promotion reserves the right to make changes to these regulations at any time, without giving any reason, by informing the Participants by publishing the amended regulations on medicadent.com.pl.
5. The Organizer of the Promotion reserves the right to change the period of the Promotion or its total cancellation without giving any reason by posting the appropriate information on medicadent.com.pl.
6. .Changes in the regulations and the end of the Promotion cannot infringe the rights acquired by the Participant until the end of the Promotion.
7. These regulations are the only document defining the rules of the Promotion. Only the provisions of these regulations have legal force, and all advertising and promotional materials concerning the Promotion are for informational purposes only.
8. In case of doubts related to the Promotion, the Participant can directly contact any Medicadent facility to get answers to all questions.

§3

1. All complaints related to the course of the Promotion, along with the justification, can be submitted by Participants only in writing to the Organizer’s address: Medicadent Dentistry Piątkowska No. 110A loc. U2, 60-649.
2. Submitted complaints will be considered by the Organizer promptly, but no later than within 14 days from the date of receiving the complaint.
3. Interested parties will be notified about the result of the complaint consideration in writing, to the address provided by the complainant, no later than within 7 days from the date of consideration of the complaint.

§4

1. In matters not regulated by these regulations, the provisions of the Civil Code apply.
2. All disputes related to this Promotion will be settled by the competent local common court.
